The Personalized program does not use your precise location, web browsing data collected as an internet service provider, information about your medical conditions, financial account information, or Customer Proprietary Network Information. It also does not access or use the contents of your texts, emails, or calls.

You or your authorized agent may opt out of the sharing and selling of your personal information and opt out of the Personalized program by completing and submitting this form or by calling us at 844-246-1628.
