Replace a lost or stolen AT&T Promotion Card

If your AT&T Promotion Card has not expired, contact the Rebate Processing Center to cancel the lost or stolen card and request a replacement.

Request a replacement rebate AT&T Promotion Card

Details

If your AT&T Promotion Card is lost or stolen, call the Rebate Processing Center using the phone number that applies to your card. The Rebate Processing Center will cancel the lost or stolen card and mail you a replacement for the remaining balance, if applicable.

Information necessary to receive an AT&T Promotion replacement card
You must provide the Rebate Processing Center with the following information that can be found on your copy of the rebate form submitted:
  • Name on your AT&T Promotion Card
  • Address associated with your AT&T Promotion Card
  • Wireless number associated with your AT&T Promotion Card
  • The IMEI number of the device associated with your AT&T Promotion Card
  • Purchase date of the qualifying device

Important information to know before calling the Rebate Processing Center
  • They must speak with the person whose name appears on the lost or stolen AT&T Promotion Card.
  • They will only reissue a replacement AT&T Promotion Card to the person whose name appears on the lost or stolen card.
  • They will not replace an expired AT&T Promotion Card.
